i'm gonna analyze this, the boy is likely to be from an rich family, he's subtilty arrogant, selfish, self-important and not used to cold faces, overall an unpleasant person. the woman is kind, and can't refuse things, while also being over worked, overall, she an likable person who can tell what someone wants, and won't refuse, but she'll show her displeasure with her cold face and words, but action speaks louder then words. the boy is arrogant and selfish is because, when she was choking or had to remove the semen, rather then show concern for her, he is wallowing is self-satisfaction, and he shows his displeasure when his waste product is being disposed of, showing his selfishness, and arrogance. his feelings of self-importance was also shown when he wanted more, being unsatisfied by the nurse who acquired specialized equipment, likely of her own money because of her kindness for just one patient. the boy is also likely to be highly sheltered, as he gets offended over nothing.
