::facepalm:: This is like jr. high reading level, the author just wanted to tell the reader, even when MC got what he wanted by drugging her, he still felt empty since he had no interaction at all with her. He did not know her, nor did he understand what she is like outside of her role as the doctor for his school. Which is why "he did not know why she smiled". The act itself was truly sad since MC actually had feelings for her, not just lust.
