Okay I deleted my previous comment so from what I can understand the witch was kidnapped and become a sex slave, she managed to escape but was somehow captured again but not before she killed the guy with the horn on his forehead. that's why there's a skull with a horn on the forehead. also for the ending, I actually have no idea, she probably got stuck like that because the only guy who can remove it died.
