I could be wrong, but, it seems like Senpai has a continued interest in him, but he keeps looking at others, instead of him, even after high school, she keeps inviting him to see her, even after he keeps rejecting her invitations, and in the flashback at the end, where he thinks Senpai isn't interested in him, she clarifies that isn't it, and when he thinks that he can't get a girlfriend because his ex-girlfriend said that they're friends, Senpai tells him, don't ever give up, do your best, and finally at the end, he wonders about how Senpai, never gave up on him, and continues to do her best, to get him to look at her differently, despite them being friends, ever since high school.

My take on this is that they were close back in HS, he very obviously had feelings for her but she likely just thought of them as friends, he moved on with his one sided love, got a gf in college or wtv, but shit happens cant get it up. They fuck his emotions are going haywire but for her this was just sex, so shes gone again and hes left confused and with mixed signals. Also this mf "i cant cheat just like that" seconds later hes deep in there, lmao
