In response to the previous commenter, I would say that the artist actually does a fantastic job of conveying the deep contrast between the characters' individual personhood, and the subsequent removal of all agency that allows her to act upon that personhood. It is subtle, and perhaps open to argumentation, but the tell is in the art style. Notice that her two friends are actually more attractive than she is, even though the artist endeavors to draw everyone in those early panels as simplistic caricature, thus nearly eradicating all sex appeal; this is the point. He then takes the least attractive girl among the three friends, who is drawn in a completely non-sexual style, and puts her in this horrific situation where she acts realistically. The deep, disturbing contrast between her entire design, and the art style used to render her, and the disgusting, evil, adult act to which she subjected, well. . . Therein lies the poignancy.

Emphasising the victim's agency is important in stories like this because it's first and foremost dealing with characters. It's a cautionary tale, so that element is easily covered (actions have consequences), but it also requires an emphasis on what is lost. Natsuko is clearly losing something sacred which is her bodily autonomy, but the early pages don't express its value. Creating a moment that could establish its importance in a manner that is explicit could serve as a meaningful contrast to the later pages. I think the fact that this was intended to be pornographic justifies the explicit imagery on its own, but I think there's other more gratifying ways to do so as well.
