I haven't read all the manga chapters but I believe in the animation, it has been mentioned several times that MC (the guy) is wondering is it real or is all just in his head/his imagination. Well, anyway, if it's all real then at least it has a happy ending... If it's all just in his head, then this is how you make NTR without actually any NTR (though it's really a cheap and lazy plot)... I believe it's the author intention that readers are left wondering if the NTR are all real or just imagination.

It looks to me that only the last chapter is what's actually happening, while the rest of the chapters are mixed in with Ichina's imagination.

MC imagined the whole thing as a "what if" scenario concerning the drugs...This type of full of plot holes
